Application for Care The following are documents and forms to print out, read and sign. Save time on your first visit by downloading them, filling them out, and bringing them with you to your first appointment. If you have any questions, please ask the Office Manager when you come in. ** These forms are created to be filled out while online for your convenience. Click on the line to be filled in, or if a descriptive answer is required, click just below the question to get a cursor. Information is NOT sent anywhere. Once you close the file, any changes to the form are lost, however, you do have the option to save the forms locally to your hard drive.
